Hospice Care

We respect your wishes and will develop a safe care plan that keeps familiar surroundings a priority. Here are all of the places that we provide hospice care to you or your loved ones: your own home, the home of a family member, a retirement or assisted living apartment, or a nursing facility.

Our hospice services are covered as part of your Medicare Benefit. If you have other insurance, our staff will notify you of your responsibility prior to the beginning of services.

Care team

Hospice is a special way of caring for people with terminal illnesses as well as their families. At Westminster at Home, we provide expert and compassionate physical, emotional and spiritual support and care through a physician-led interdisciplinary team of nurses, aides, therapists, a social worker, chaplain and volunteers. We are there to help you and your family develop a personalized care plan so that you can continue to live life to the fullest – and to bring peace during this time of transition.

Our team is always available to answer your questions about hospice care.

Patient Care

Registered nurses and certified nursing assistants provide care visits in your home based on your changing levels of need.

Chaplain

Hospice chaplain service is optional and non-denominational. Hospice supports the belief that each of us has an important spiritual side and provides support to people of all faiths to help meet spiritual needs. At your request, our hospice chaplain is available to facilitate your wishes and act as a spiritual liaison to your religious community.

Medications

Hospice covers the cost of all medications related to a terminal condition as well as incontinence and other medical supplies. We arrange the delivery and setup of medically necessary equipment in your home.

Emotional Support

Our social worker provides a wide range of services and support, depending on each patient and family’s needs. Our 13-month bereavement program ensures that your family and loved ones will have the emotional and spiritual support they need.

Inpatient & Respite Care

Westminster Canterbury offers private rooms for inpatient and respite care.

Pain & Symptom Management

We want to make you as comfortable as possible. Our physician and nurses work with you to tailor an effective plan to manage pain and other symptoms.

Volunteers

Volunteers are the heart of hospice. After extensive training, volunteers are a reassuring and comforting presence when a family needs to rest. Hospice volunteers perform a wide variety of tasks and services to support patients and families — from picking up medication to simply sitting and listening.

* We encourage surviving family members to wait a minimum of one year following the death of a loved one to become a hospice volunteer.
